How do I know if an updated sports physical is needed?

Sports physicals are valid for three years (from the date of the original office visit). When the physical date has reached its expiration, a visit the doctor is needed to submit a new physical form.

Please note: Our system will not send you a notification to let you know that the physical has expired.

If you find that you cannot complete an activity or athletic registration, it's likely the physical on file is invalid, missing or expired.
